Waiver and liability agreement THE ROYAL VAULT BY WESTSIDE NUTRITION — ASSUMPTION OF RISK, RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT

1. ACKNOWLEDGEMENT OF RISK. I understand that physical exercise, strength training, group fitness classes, personal training, and the use of exercise equipment involve inherent risks, including but not limited to muscle strain, sprain, fracture, cardiovascular events, heat illness, and in rare cases serious injury or death. I voluntarily choose to participate with full knowledge of those risks.
2. PHYSICAL CONDITION. I confirm that I am physically able to participate in exercise activity, or that I have consulted a physician and been cleared to do so. I agree to stop activity and notify staff immediately if I experience pain, dizziness, shortness of breath, or any other warning sign.
3. NUTRITION PRODUCTS. I understand that beverages and supplements sold at Westside Nutrition are not medications and are not intended to diagnose, treat, cure, or prevent any disease. I am responsible for disclosing any allergy, medical condition, pregnancy, or medication interaction before consuming any product.
4. RELEASE. To the fullest extent permitted by law, I release and hold harmless The Royal Vault by Westside Nutrition, its owner, employees, independent trainers, contractors, and landlords from any claim arising out of my participation, except for claims arising from gross negligence or willful misconduct.
5. PERSONAL PROPERTY. The facility is not responsible for lost, stolen, or damaged personal property.
6. MEDIA. I understand that photos or video may be taken in common areas for promotional purposes, and that I may opt out at any time in writing.
7. RULES OF THE HOUSE. I agree to re-rack weights, wipe down equipment, respect staff and other members, and follow posted facility rules. Harassment of any kind results in immediate termination of access.
8. MEDICAL TREATMENT. I authorize staff to summon emergency medical care on my behalf if needed, and I accept financial responsibility for that care.
9. TERM AND SEVERABILITY. This agreement remains in effect for the entire duration of my relationship with the facility. If any part is found unenforceable, the remainder stays in force.
10. ELECTRONIC SIGNATURE. I agree that typing my full legal name below constitutes my legal signature, and that the date, time, and version of this agreement are recorded with it.
By signing, I confirm I have read this agreement in full, I understand it, and I am signing it freely.
